I hereby acknowledge receipt from Vashon Island Pet Protectors (VIPP) of the animal described above. I understand that VIPP can make no guarantee in regard to the condition or ownership of the animal and that VIPP is supplying all of the information VIPP has in their possession regarding the animal.

Terms & Conditions*
- I am adopting the pet described below through Vashon Island Pet Protectors aka VIPP. The pet is being adopted as a companion and will live as a family member with me inside my home. I will not allow the pet to be used for purposes of vivisection or experimentation.
- I will provide proper and sufficient, food, water, shelter and kind treatment for the pet at all times. I further agree the dog will not be chained at any time as a means of restraint or be a primarily outside dog.
- I agree if I am unable to care for the animal and unable to rehome the animal on my own, VIPP is available for consultation on returning animal to VIPP.
- I will keep a collar and identification tag on the dog at all times and will license the dog with the county/city.
- I agree to immediately notify VIPP if the pet is lost or stolen. I further agree, in the event the animal is lost, that I will make every reasonable effort to attempt to locate and claim the animal, and to immediately retrieve the pet from any public pound or animal shelter when notified the animal is being held at any such location and to pay any costs associated with its recovery. I will also notify VIPP's Lost & Found as well.
- I understand that I am entitled to a free wellness exam at Fair Isle Animal Clinic or 4 Paws Veterinary Clinic if I schedule it within one week of the adoption date.
